Township Assistance Application

In order to receive assistance from the Jeffersonville Township Trustee, the following requirements must be met:

1.  Reside in Jeffersonville or Clarksville.
2.  All household members over the age of 18 must complete and sign a Township Assistance Application.
3.  All household members over the age of 18 must submit requested documents. We have provided lists of general documents required.
4. A valid email address must be included on page 4 of application in the notes section.

Listing of General Documents Required:

Please note that additional documentation may be requested (e.g., doctor’s note, receipt for large expense).
5.  After submitting application, referrals(if available) will be provided to other agencies offering assistance. 
Proof of application to these referred agencies will be required as part of application process.
